Your personal information and medical problems are kept confidential. Please remember that the age of medical consent is 16 in Ontario. Files of children under 16 are kept with their parents. All 18 year olds have their own medical record charts. It is often a tricky situation as to how to convey information between family members.

What is important is to keep your ADDRESS and PHONE NUMBER updated in our system. Situations arise where we cannot contact you because of out-dated information.

Your health card is probably the most valuable document you own. It opens the door to vast health expenditures on your behalf when you need it. Other times, it tracks your journey through our system. Without it, you will usually have to pay the costs of your health care up front and then try to recover it later.

It will probably be the cornerstone of the electronic tracking when it becomes integrated. Of interest, is that we now receive notification when you use your health card outside of the MISSISSAUGA FHO. It may feel uncomfortable being tracked, but we enjoy a level of care envied around the world. If we don’t look after it, we will lose it. Report any changes to your number or loss of card immediately.
